Course Application Procedures and Guidelines:
I. Enrollment
a. In Person:
On completion of the application form, please return it either in person or by an authorized representative to Hong Kong St. John Ambulance Association, 4/F., St. John Tower, 2 MacDonnell Road, Hong Kong. Office Hours: Mondays to Fridays from 8:30am to 6:18pm (except Saturdays, Sundays and Public Holidays).
b. By Mail:
Please complete the application form along with a crossed cheque made payable to Hong Kong St. John Ambulance for the course fee and relevant items (if any), a self-addressed stamped ($2) envelope; and then mail to Hong Kong St. John Ambulance Association, 4/F., St. John Tower, 2 MacDonnell Road, Hong Kong. (PLEASE DO NOT SEND CASH / PROMISSORY NOTE). The application will be processed in about two weeks. The application will not be accepted if the information in the application form is incorrect, incomplete or insufficient, the applicant has sent insufficient fee or the applicant’s age does not meet the enrollment requirement. All application documents will then be returned to the applicant. Hong Kong St. John Ambulance is not responsible for any loss. When a class is full or cancelled by the Association, a 100% refund will be made.
c. By fax or email:
If the applicant would like to pay by PPS, please complete and return the application form to the Association by fax (fax no. 2976 0457) or by email (email:assn.pps@stjohn.org.hk), please also call 2530 8020 to 2530 8024, or email to assn.pps@stjohn.org.hk for obtaining PPS payment code. The applicant must settle the payment within two working days after receiving the PPS payment code; otherwise, the application will not be accepted.
d. Online Payment (Suitable for some courses):
On completion of the online application form and settle the payment by VISA/MasterCard/Union Pay.
II. Admission
a. If the individual course becomes full, no further application will be accepted, all application documents will be returned to the applicant.
b. The Association reserves the right to refuse any application. The notification sheet will be sent to the applicant along with the Class and Examination Regulations. Upon acceptance of application, refund or swap course will not be accepted.
c. The notification sheet will be sent to the applicant two weeks prior to course commencement date. If applicant has not received the Class and Examination Regulations three days before class begins, please contact the Association at 2530 8020-24.
d. Applicants can arrange to have their places substituted should they be unable to attend the course by notifying the Association at least 10 days prior to course commencement with $100 Administration Fee to be charged.
e. The charge for duplicate receipt of course fee is $50.
III. Personal information of applicant
The personal data provided by means of this form will be used for course admission purposes and future contact. The provision of personal data by means of this form is voluntary. If you do not provide sufficient information, we may not be able to process your application or application documents will be returned to you. Enquiries concerning personal data collected can be sent in writing to: “Training Manager, St John Ambulance Association, 4/F., St. John Tower, 2 MacDonnell Road, Hong Kong”.
IV. Completion of Admission
a. Upon completion of admission, the applicant is admitted to be a student of the St. John Ambulance Association; he/she has to abide by the class and examination regulations including Guidelines for Course/Examination Participants in Response to COVID-19 (Please refer to Appendix). Requests on switching class will not be accepted. No refund will be made after payment.
b. In case the student has signs and symptoms of an upper respiratory infection, common cold or flu, the student should wear a face mask when attending classes.
c. Due to the large number of applications received for each course, the Association will assign classes for applicants randomly. If two or more applicants prefer to attend the same class, they should submit a joint application. Otherwise, the applicants will be admitted into different classes randomly.
d. First Aid Course Manual and Bandages Pack can be purchased at the Office of St John Ambulance Association or by mail order along with the course application (please state clearly at the back of the cheque the name of the applicant and items to be ordered). Ordered items will be delivered to the training venue on the first lesson. If the ordered items have not been claimed two weeks after course completion, all items will be disposed. No refund will be made for the ordered items.
V. Special case
a. If applicant suffers from physical or mobility constraint, i.e. knee flexion, fitted with artificial limb, visual and hearing impairments etc., and required for special assistance and arrangement while attending the course, an application form of Special Needs Arrangement upon course registration should be submitted to the Association where an administrative fee of $50 may be charged.
b. If applicant is pregnant or has a special medical condition, an application for Re-scheduling of Course / Examination by Recommendation from Registered Doctor / Registered Chinese Medicine Practitioner, assessed and approved by an attending physician or Registered Chinese Medicine Practitioner should be submitted to the Association in support of the applicant’s attending the practical training and examination.
The Association reserves the right to amend the above guidelines from time to time without notifying individual applicant. Please refer to the Association website for most updated information.
Declaration
(1) I declare that I meet the stipulated requirements of age for the selected course on the date of course commencement (I will provide my HKID Card/passport for verification if required by St. John Lecturer/Instructor/Staff). If the information given in this Form is incorrect and incomplete, I am not eligible to attend the course or being awarded a Certificate.
(2) I certify that, upon the completion of admission, I shall comply with the requirements as stipulated under the Course Application Procedures and Guidelines, including temporary arrangements, Course and Examination Notice.
(3) I understand that I need to take care of my personal belongings during class, examination and event. I certify that I am physically qualified to participate in the above course and examination. I understand that the course will involve physical exertion. I am solely responsible for any loss of or damage to my personal belongings, or any personal injury or death as a result of my own health condition, during attending course and examination. Hong Kong St. John Ambulance and the Sponsors shall not be under any liability for any loss in such circumstances.
(4) In the event of dispute in respect of these declarations or any rules and regulations arising from class, examination or event, the decision of the Hong Kong St. John Ambulance shall be final, binding and conclusive.
(5) I certify that I have read and understand the accompanying Hong Kong St. John Ambulance Guideline for the Use of Personal Data.
Hong Kong St. John Ambulance Guideline for the Use of Personal Data
Hong Kong St. John Ambulance (St. John) undertakes to comply with the requirements of the Personal Data (Privacy) Ordinance to ensure that personal data collected are accurate and securely kept. To ensure you are well informed of the personal data as collected, please read through this guideline.
Purpose of Collection and Guideline for Use of Personal Data
1. Hong Kong St. John Ambulance will use the personal data collected from data subject for the purposes for which it was collected.
2. To provide personal data to St. John is on voluntary basis. However, if you do not provide sufficient personal data, we may not be able to process your application or provide service to you.
3. St. John may use your personal data in future (name, telephone number, fax number, email address, mailing address, organization/company/school) for the purposes of providing you with information of St. John, handling application, issuing receipt, collecting feedback, announcing training courses and promoting St John activities.
4. Other than the purposes mentioned in (3), St. John will not sell/rent/transfer your personal data in any forms through any means to any other parties.
Access to and Correction of Personal Data and Request for Cessation of using Personal Data for Promotion Purposes
Apart from the exemptions provided under the Personal Data (Privacy) Ordinance, you are entitled to access and correct your personal data held by Hong Kong St. John Ambulance, and request us to cease to use your personal data for promotion purposes. However, it will not include the personal data deleted after the achievement of the purpose.
If you object St. John to use your personal data for the purposes as stated above, please mark on the “Opt-out Reply” with your full name, email address and date, and return it by mail/fax/email to us. No charge will be applied.

[Appendix] Guidelines for Course/Examination Participants in Response to COVID-19
To ensure a clean and hygienic environment for all course/examination participants, and in order to minimise the risk of contracting and spreading COVID-19, HKSJA has arranged to clean and disinfect all classrooms and training equipment on a regular basis upon resumption of courses/examinations.
All persons should follow the following precautionary measures when attending course/examination organised by HKSJA:
Before Entering into Classroom/Examination Venue
1 Scan the「Leave Home Safe」QR code and meet the vaccination requirements of Vaccine Pass when entering into classroom/examination venue.
2 Bring your own surgical mask and wear properly to cover the mouth, nose and chin throughout the course/examination.

3 Wash hands with soap and water or use 70-80% alcohol-based handrub to disinfect both hands. Repeat this step before putting on and after taking off the surgical mask.
4 Check the body temperature. Persons with symptoms of fever (forehead temperature measured by infrared thermometer higher than 37.5 oC) are required to re-check the ear temperature by a tympanic thermometer. Those with fever (ear temperature higher than 38 oC) or respiratory symptoms (e.g. cough, shortness of breath etc.) are prohibited from entering classroom/examination venue. They should inform the lecturer of the class/officer-in-charge of the examination before they leave, and are advised to consider seeking advice from a medical doctor as soon as possible.
5 The following persons are not allowed to participate in course/examination: –
5.1 Having travelled to any province of Mainland China or other overseas countries or territories in the past 14 days;
5.2 Having close contact with a confirmed case;
5.3 Are under quarantine or isolation and/or medical surveillance.
Entering into Classroom/Examination Venue
6 A floor mat sprayed with disinfectant is placed at the entrance of classroom/examination venue for disinfecting the bottom of the shoes.
7 A “Declaration Form on Health” will be issued to all persons upon entering into the classroom/examination venue. The form should be completed, properly signed and returned to the lecturer of the class/officer-in-charge of the examination immediately.
8 HKSJA reserves the right to request any participant with respiratory symptoms (e.g. cough, shortness of breath etc.) or not wearing a mask properly to leave the classroom/examination venue.
9 Report to the lecturer of the class/officer-in-charge of the examination immediately if feeling unwell during the course/examination.
10 Should not take off the surgical mask throughout the course/examination.
11 Replace the surgical mask immediately if it is stained or wet.
12 Use 70-80% alcohol-based handrub to disinfect both hands, and follow all other preventive measures according to the instruction of the lecturer/examiner before each practical session. Bring and wear your own latex gloves (limited supply can be provided by the lecturer of the class/officer-in-charge of the examination upon request, while stock lasts) to avoid direct body contact.
Other Arrangements
13 When conducting CPR training/practical examination, while most practical procedures will remain unchanged, there is no need to do mouth-to-mouth rescue breaths. You are only required to state out the application of this procedure.
14 All used surgical masks or latex gloves should be disposed into designated lidded rubbish bin.
*15 Course/examination participants who are unable to meet the vaccination requirements of Vaccine Pass are required to conduct a Rapid Antigen Test (RAT) every day before attending class/examination. Course/examination participants with negative test results will be allowed access to St. John training premises.
For the above-mentioned eligible course/examination participants, please mark your full name and test date on the RAT Test Cassette and take a photo for record. You are required to present the mentioned photo to St. John Staff before entering the classroom/examination venue.
*Remarks: Starting from 11 May 2022, all participants have to comply with the Vaccine Pass requirement for attending class/examination at St. John training premises. Except for those course/examination participants who have successfully registered for a course on or before 24 January 2022 but have not completed the course/examination.

報讀課程方法及有關條款:
I. 報名方法:
a. 親自報名:
請攜同報名表格於辦公時間內親臨或委託他人前來「香港麥當勞道二號四樓,聖約翰救傷會辦事處」辦理。 辦公時間:星期一至星期五上午八時三十分至下午六時十八分(公眾假期除外)。
b. 郵寄報名:
申請者請填妥報名表格,連同以「香港聖約翰救護機構」名義抬頭之劃線支票及貼有郵票($2)之回郵信封,寄香港麥當勞道二號四樓聖約翰救傷會收。【請勿郵寄現金 / 期票 / 本票】
郵寄報名手續處理時間一般約兩星期。資料若不齊全、錯誤、付款不足或不符合入學資格者,本機構將不會接受報名及退回原件或支票。若因郵遞錯誤,本會恕不負責。如郵寄報名之課程組別滿額或取消,本會稍後將退回有關款項。
c. 傳真或電郵報名:
如申請者欲以繳費靈(PPS)付款,請將填妥的報名表格傳真(傳真號碼2976 0457),並需致電2530 8020至2530 8024 或將表格電郵(email: assn.pps@stjohn.org.hk)索取繳費靈(PPS)的付款號碼。申請者請於收到付款號碼後兩個工作天內繳費,否則名額將被取消。
d. 網上報名(適用於部份課程):
申請者請填妥網上報名表格,以VISA/MasterCard/銀聯付款。
II. 取錄事宜:
a. 若課程因滿額,本機構將不接受報名,並將原件退回
b. 本會有權不接受任何報名。取錄與否,請以本會寄出之學員上課及考試須知(正本)作實。已被接納的申請,恕不接受辦理調班及所繳的費用概不退回。
c. 本機構將會於開課前兩星期內以書面通知申請者取錄與否。如申請者在課程開課前三天,仍未收到本機構的「學員上課及考試通知」,請電郵致assn@stjohn.org.hk查詢有關課程申請事宜。
d. 於課程開課日十天前或以上,學員可提出轉名申請,費用為港幣一百元正,逾期恕不接受辦理。
e. 若申請重發收據,費用為港幣五十元正。
III. 個人資料:
申請者提供之個人資料,將作辦理課程登記之用,印製證書或日後聯絡之用。申請者所提供之個人資料純屬自願。如資料不充份或不正確,本會可能無法或延遲處理你的申請,並將申請寄件原件退回,若有查詢或更改有關你的個人資料,申請者需以書面寄「香港麥當勞道二號四樓,聖約翰救傷會辦事處」,通知本機構。
IV. 上課:
a. 申請者手續一經辦妥及取錄後,申請者成為本機構學員,學員必須遵守本機構一切有關上課守則和規定,包括2019新冠肺炎期間上課及考試安排(見附件),學員未能完成課程,所繳的費用概不退回。
b. 學員如有上呼吸道感染、傷風或感冒徵狀,請自備口罩上課。
c. 由於報名人數眾多,本會將會使用不同課室上課,如同時有兩位或以上學員一起申請同一班,請把所有表格收集後一同遞交,否則會跟報名先後次序編排在不同班別上課。
d. 申請者購買急救課程手冊及繃帶包,可於辦公時間親臨本會購買或於郵寄申請表同時以支票訂購(不設上課後補購),並請於支票背面清楚填上申請人的姓名及所需訂購的用品及聯絡電話。所有訂購物品,將於第一天上課時派發。如課程完結日期兩星期內仍未到救傷會領取者,所有物品將被報銷。所有已付金額,將不會退還。
Ⅴ.特殊情況:
a. 申請者如因肢體活動受阻或有限制,例如︰膝部不能屈曲、裝有義肢、視障、聽障等等,而需要救傷會作出特別協助或安排,必須於報名前呈交 「特殊需要」安排申請表及相關醫療文件的副本,經本會批核後,將盡力予以協助,並將按情況收取行政費用港幣五十元正。
b. 學員如因懷孕或特殊醫療狀況,需得主診醫生/註冊中醫評估及批核,該學員合適進行實習課及考試,並將「課程或考試延期申請表 - 醫生/註冊中醫証明書」交給主診醫生/註冊中醫填寫並呈交本會辦事處申請。
本機構保留修改上述條款之權利,無須事前通知。請瀏覽本機構網頁獲取最新資料。
聲明:
(1) 本人特此聲明,本人的年齡及資格,在報讀課程開班的首天已達課程所需要求【導師/職員有權查核本人的香港身份證或護照】,若資料不符,本人明白將不能參加課程及不獲頒證書。
(2) 本人同意一經取錄成為學員,會遵守上述有關條款,包括臨時安排及上課守則和規定。
(3) 本人明白在上課、考試及活動期間,本人須注意個人及財物安全。本人明白申請之課程涉及體力操作。本人的健康狀況適宜參加上述課程及考試。倘於上課、考試及活動期間發生財物損毀、遺失,或因本人的健康狀況而引致意外,包括死亡、受傷,本人將自行承擔一切責任。香港聖約翰救護機構、贊助機構,無須負上賠償或任何法律責任。
(4) 倘於上課、考試及活動所引發的任何爭議(包括本聲明或任何規則的解釋和行使) ,香港聖約翰救護機構將擁有最終的決定權。
(5) 本人確認已閱讀及明白隨本表格附上有關香港聖約翰救護機構的個人資料使用守則。

香港聖約翰救護機構盡力遵守及執行《個人資料(私隱)條例》中所列載的規定,確保儲存的個人私隱及個人資料處理得到充分保護,準確無誤,及有妥善的儲存方法。為確保你能充分了解向本機構提供個人資料的準則,請細閱本守則。

1. 香港聖約翰救護機構將依照在收集資料時所說明之目的去使用該等資料。

除了《個人資料(私隱)條例》規定的豁免範圍外,你有權就香港聖約翰救護機備存有關你的個人資料作出查閱、更改及停止使用你的個人資料作推廣用途的要求,但已達成使用目的後而刪除的個人資料除外。

[附件]2019新冠肺炎期間上課及考試安排指引
為確保所有學員能於清潔的環境下安心復課,並降低感染和傳播2019新冠肺炎病毒的風險,香港聖約翰救護機構已安排加強對所有課室及訓練儀器的清潔及消毒工作。
所有參與香港聖約翰救護機構課程/考試的人士,須遵守以下之預防措施:
在進入課室/考試場地前
1. 在進入上課/考試場地前,需掃描「安心出行」二維碼及持有「疫苗通行證」。
2. 在課程/考試期間,正確地戴上自備的外科口罩(口罩須完全覆蓋口、鼻和下巴)。

3. 以梘液和清水清洗雙手、或以70-80%酒精搓手液潔手。在佩戴口罩前及除掉口罩後,須重覆此程序。
4. 量度體溫。如在使用紅外線額探後溫度高於攝氏37.5oC人士,須再接受紅外線耳探。耳探溫度高於攝氏38oC、或有呼吸道感染徵狀(例如:咳嗽、氣促等)的人士,不可以進入課室或考試場地。學員在離開前,須先通知課程講師/考試主管,並儘早求醫。
5. 以下人士不可以參與香港聖約翰救護機構舉辦的課程/考試︰
5.1 於過去14日內曾到訪內地任何省市或海外國家/地區
5.2 與確診人士有緊密接觸
5.3 正接受政府的強制檢疫或隔離或/及醫學監測
進入課室/考試場地
6. 在課室/考試場地的進出口位置,已放置了已噴灑消毒劑的地氈,讓學員在進場時,消毒鞋底。
7. 當進入課室/考試場地,學員會獲發「健康申報表」。學員必須填妥、簽署,並交回課程講師/考試主管。
8. 如發現學員有呼吸道感染徵狀(例如:咳嗽、氣促等)、或未有正確地戴上自備的外科口罩,香港聖約翰救護機構保留要求有關學員即時離開的權利。
9. 如在上課/考試期間有任何不適,須即時向講師/考試主管報告。
10. 在上課/考試期間,不可除下口罩。
11. 如外科口罩於上課/考試期間沾污或沾濕,須即時更換。
12. 在進行實習課/實習考試前,須按講師/考試主管指示使用70-80%酒精搓手液清潔雙手,以及配合其他防疫措施。在進行實習課/實習考試期間,應戴上自備的乳膠手套,以避免直接身體接觸。如有需要,可向講師/考試主管索取(數量有限)。
其他安排
13. 在進行心肺復甦法實習/考試時,將以口述代替進行「口對口人工呼吸」步驟(不用吹氣),其他程序則維持不變。
14. 所有用過的口罩/乳膠手套須棄置於口罩及手套專用的有蓋垃圾桶內。
*15. 如個別學員尚未持有「疫苗通行證」,有關學員必須每次於返回本中心上課/考試當天進行快速抗原測試,並取得陰性結果,方可進入各培訓中心。學員須在有結果的測試劑面寫上其姓名及檢測日期,把相關照片保存在手機內,以及於進入課室/考試場地前展示有關相片予本機構職員查閱。
*備註:只適用於2022年1月24日前已報讀課程但仍未完成課程或考試之學員。由2022年5月11日起,所有學員須持有「疫苗通行證」,方可進入本中心上課/考試(獲醫學豁免情況除外)。
